eScreening is a customizable data platform which allows patients to submit health information remotely from their personal devices directly to the medical record.
Customizable and standardized forms can complete clinical reminders, create unsigned editable text notes, generate health factors, and increase the quality of patients’ time with their provider by reducing time spent on mandatory documentation.
Already a proven assessment tool, eScreening was pioneered for use by the V.A., America’s largest healthcare system. This tool has been in extensive use with Veterans to help identify at-risk patients in need of additional care and resources.
The eScreening assessment modules are fully customizable based on a program’s specific clinical needs.
Patients receive an email to conveniently complete electronic screenings on their personal device. Once the screenings have been completed, eScreening generates a text note in their chart, satisfies clinical reminders, populates alerts, creates health factors, and pushes data to the EHR for measurement-based care.

Enhanced Orbits
Beginning in 2024, orbits can now have an eScreening capability to allow each patient to answer relevant questions on their condition and needs that then allow their orbit to automatically evolve to address any incremental guidance and empowerment needed, as well as alert care teams to emerging patient or family needs – helping them address smaller issues before they become larger ones.
